Personal Health Solutions concentrates clinical resources where they will be most effective. We use a targeted approach focused on high impact areas which helps promote cost-effective quality care. Through our evidence and experience-based programs, we focus on areas where we can make the most significant impact through precertification, complex case management, catastrophic case manage and specialty case management programs. Beginning with the initial hospital admission, a team of dedicated case managers and physicians help support a plan for the patient's next level of care. They also work with the patient and family to identify any support the patient may need after leaving the hospital. This early intervention helps to reduce in-hospital days and prevent readmission.
