If your employees are dealing with heart disease, you know there's a lot to it. So we've made sure that the CareAllies Disease Management Program offers a lot of support and a lot of helpful information.

Knowledge is power—especially when you're managing heart disease. But that's not all it takes. You've got to have the right kind of support. That's why everyone who joins the Disease Management program is assigned a nurse—someone who has extensive training in heart disease issues. The nurse will work closely with your employees to keep their heart healthy—doing things like:

  • Reviewing health records and test results to give your employee information that's relevant to them.
  • Reminding them that they are due for important health tests, exams and checkups.
  • Helping to find additional educational information and helpful resources.
  • Working to help them and their treating providers to create a cardiac action plan.

After they've enrolled, they'll see what an impact the Disease Management nurse can have on their cardiac health—helping them learn the best ways to get their heart in the best condition possible.

For someone diagnosed with heart disease, it's a good idea to learn how your heart works—and what they can do to make sure it works better.

Use the cardiac workbook to track progress.

Once they've enrolled in the Disease Management cardiac program they'll receive a cardiac workbook that will prove to be an essential resource for them—and their heart.

The Health Coach and Disease Management cardiac nurse

The Disease Management nurse isn't the only one at CareAllies working to keep you healthy. Instead of concentrating solely on your employee's cardiac health, the CareAllies Health CoachSM takes a total view of health issues.

Should your employees have any heart-related hospitalizations, they'll be assigned a Complex Case Manager, someone who helps them access care from discharge through recovery. When necessary, the Health Coach will work with the Complex Case Manager to help ensure the best service possible.
