Reaching Out to Those in Need
CareAllies Health Advisors proactively identify and reach out to members who are at risk for significant health issues but are not eligible for chronic or acute disease or case management programs. Our team of health and wellness professionals includes registered nurses, behavioralists, health educators and nutritionists, supported by doctors and pharmacists. Using the team approach, our health advisors reach members earlier in their health care experience and provide valuable health and wellness coaching and resources.
One Powerful Program
This program increases early identification and proactive engagement of the at-risk population through completion of a Health Risk Assessment; predictive modeling; and customer service identification and direct-to-member communication that heightens awareness of the program and promotes self-identification. Once contacted, health advisors assess each member's overall health status, health attitudes and behavior, and readiness to change. We then assign a resource from our team that best matches each individual's needs.

- Integration with our Well Informed program, offering valuable information and promoting appropriate, seamless care.
- Preference-sensitive coaching techniques, allowing our nurses to coach members to successful clinical outcomes more often.
- An integrated care management system, enabling nurses to track and manage all member health care activities: medical, pharmacy, behavioral, disability and more.

An Example of How it works:
Jane Doe has osteoarthritis of the knee, and her doctor has told her that she may need knee surgery. Jane calls the Health Advisor program seeking advice on her treatment options. Jane's Health Advisor provides her with treatment decision support and pertinent information regarding her treatment choices, in the benefits and risks. Jane's Health Advisor also provides her with provider cost and quality information as she considers surgery. After consideration, Jane goes back to her doctor with some additional questions. Jane decides to use drug therapy (simple pain relievers) with some outpatient physical therapy to treat her osteoarthritis as opposed to having surgery.
John Doe has high cholesterol, is sedentary, and has lots of stress in his life. John completes an HRA that identifies all these risk factors, and indicates that his HRA score is well below average for his age. Based on these HRA results, a Health Advisor speaks to John about his risks and provides him with information on how to reduce his cholesterol to a healthy level (which includes increasing physical activity). The Health Advisor also informs John of the stress management program available to him and transfers him for enrollment at the end of their conversation.
