For major health issues, our Case Managers ensure patients get the level of care that's right for them.

Most of the time, a trip to the doctor means dealing with minor issues like the flu or allergies. But should something more serious come up, isn't it comforting to know that your employees won't have to go through it alone? CareAllies' Case Management program is designed to give patients just the right amount of support and assistance at the right time. A dedicated Case Manager will be assigned when short-term and/or outpatient care is required. If a patient is dealing with multiple medical conditions or recovering from a serious condition that may have long-term health implications, a Complex Case Manager will be there to provide assistance. For the most difficult medical conditions - like cancer or an organ transplant - a Specialty Case Manager can support the patient through the whole experience.

 

Help Contain Health Risk Gaps

Predictive Modeling uses a unique combination of information from medical, pharmacy and behavioral claims; clinical laboratory results; physician interaction; and inpatient utilization review to identify individuals who may have a health risk or a gap in care, or who are likely to incur medical costs greater than $10,000 in the next 12 months. Using industry-leading technology, we project a risk level for every employee and dependent. Our nurse case managers then proactively reach out to those identified as being at risk to help them take the appropriate steps to improve their health. This early intervention means better health for your employees and their family members, fewer employee sick days and lower medical costs for you. Predictive Modeling is available as an option with Personal Health Solutions for an additional charge.

 

Engage a Case Manager to help ensure a smooth recovery.

Case Managers are assigned before a surgery that requires in-home or outpatient care following the operation - knee surgery, for example. Even before a patient enters the hospital, they'll learn what an asset the Case Manager can be. In addition to helping them understand what to expect before, during and after surgery, a Case Manager will:

 

In addition, the Case Manager will interact with health care providers to help ensure patients are well versed about their condition, the surgical procedure, possible complications and more.

Plus, the Case Manager isn't the only one who can provide assistance. A patient can always contact the 24-Hour Nurse Line if something comes up in the middle of the night or on a weekend if this offering has been selected.
